Enrollment Forms

Enrollment is only available to students who currently have a position or who have accepted a position in a class.  For lottery information, please click on our Lottery Tab.

Enrollment forms for 2018-2019 will be available in April 2018
Please read the complete letter below for enrollment instructions. 

 

Enrollment forms can be mailed or emailed to our school office (except for the ORIGINAL birth certificate, which must be brought into the office to be copied).  Forms can also be hand-delivered to the school office.  Pictures of forms WILL NOT be accepted.  Our address is 278 S. Enrollment Forms Required:

NEW STUDENTS

New students will need to provide the following items before the beginning of school:

·         Enrollment Form:  One form per family.     

o    Please include all siblings on the same form.

o    This form is used for all contact and emergency information for Falcon Ridge.  Please make sure information is correct.

o    Please list all people who have permission to transport your child(ren) from school.  They can either be listed as an emergency contact or in the space provided on the form.

·         Student Handbook Acknowledgement:  One form per student. Please sign and have student sign.  A copy of the Handbook can be found on our website.

·         Bus Request Form:  One form per family.

·         Statewide Home Language Survey:  One form per student.

·         Standard Student Residency Questionnaire:  One form per student.

·     Immunization Record or Immunization Exemption Form:  Please be aware of the Idaho law immunization requirements (Please see the 2017-18 Immunization Parent Guide School Requirements for more information.)

·         Birth Certificate:  Original copy, we will make a copy in the office.

               ·         Proof of Residency: Current utility bill, rental agreement, mortgage statement  or other municipal document                                 showing a current address. (Driver's license, car registration, etc., are not acceptable.)


FOR RETURNING STUDENTS:

·         Enrollment Form:  One form per family.     

o    Please include all siblings on the same form.

o    This form is used for all contact and emergency information for Falcon Ridge.  Please make sure information is correct.

o    Please list all people who have permission to transport your child(ren) from school.  They can either be listed as an emergency contact or in the space provided on the form.

·         Student Handbook Form:  One form per student.  Please sign and have student sign.  A copy of the Handbook can be found on our website.

·         Bus Request Form:  NEW students needing bus service will need to fill out a bus request form (one form per family.)  RETURNING students—bus forms can be picked up in the office.  The form ONLY needs to be filled out if there is a change in address or different pick up/drop off location.

·         Statewide Home Language Survey:  One form per student.

·         Standard Student Residency Questionnaire:  One form per student.

·         Attention 7th Graders:

o    Immunization Record or Immunization Exemption Form: We must receive an updated immunization record.  Idaho law requires that all 7th graders, in addition to the school entry requirements, must receive 1 dose of Tdap and 1 dose of Meningococcal vaccine prior to the start of school. (Please see the 2017-18 Immunization Parent Guide School Requirements for more information.)
